Once we receive your referral, you will receive an acceptance or decline notification. We try to accept as many individuals as possible within clinic capacity.
If accepted, you’ll receive a registration link to complete your initial intake forms.
We recommend reviewing our website in advance so you know what to expect.
Your first appointment is typically a 20-minute phone or video visit with Dr. Motamedi to:
If you already have a documented ADHD diagnosis, you may be eligible for physician-led clinical assessment (MSP-covered).
If diagnostic clarification is needed, psychometric assessment (private-pay) may be recommended.
You may receive:
Occasionally, questionnaires are not required, and care can move directly to management.
Completing questionnaires promptly helps avoid delays.
You’ll receive an email with details about your next appointment, depending on your care pathway.
Psychometric assessment:
Azadeh Rezazadeh, RCC, will contact you to schedule a ~2-hour Zoom interview (with breaks and flexible timing).
Physician-led assessment:
Your appointment will be scheduled directly with Dr. Motamedi (secure video).
Regardless of pathway, Dr. Motamedi reviews all information, discusses a working diagnosis, and reviews treatment options. Diagnoses may evolve over time.
